All the forms needed for the use of human and animal subjects, and assurance compliance are now available in MSWord  and Adobe formats. These word processing documents contain the latest revisions to meet local and external requirements. It's new, quick, and easy! Most of you use a word processor everyday; The formatted forms work just like any other document. Just open up the document, type in your responses and print it. You can save each form so you’ll be able to go back and update the form(s) for new or resubmission.

System requirements:
Windows-Based Systems

Microsoft Word version 8.0 or greater
Adobe Acrobat Reader 7.0

We urge you to use these in-place of the traditional mail system.
If you encounter any errors, please E-mail David Compton at David_Compton@pba.edu with a description of the error.

BEFORE YOU BEGIN!!! Have you configured your Netscape WEB browser to handle MSWord documents? How can you tell? If you click on one of the files below and you view text in the WEB browser, then NO, your WEB browser is not configured.
The easiest thing to do is to save the file to disk;
1. Right Click on the FORM of your choice
2. The SAVE AS prompt box appears. Save the file to a directory of your choice.
3. Startup Microsoft Word  or Adobe Acrobat and open the document you just saved.

In addition to the word processing formats, copies of the application pages, in Adobe Acrobat (PDF) format, may be accessed below.

HELP AND INSTRUCTIONS are available for downloading and processing (printing and viewing) Acrobat files. Acrobat Reader software must be obtained in order to view and print Acrobat files.
